Jordan is a caring and compassionate social worker with experience working with young adults, middle aged adults and seniors in various capacities. Jordan’s passion for working with older adults stem from her volunteer time in high school at a neighborhood long-term care facility. During that time, Jordan witnessed numerous instances of abuse and neglect. From that moment, Jordan knew that advocating for vulnerable adults to ensure they are cared for properly would be her career objective.

Jordan has over ten years of experience working in healthcare. Jordan worked as a secretary at a law firm specializing in Guardianship among other areas for over six years. After completion of her Bachelor Degree in Psychology at Purdue University, Jordan worked at a care coordination unit under IDOA in the western suburbs conducting Interim and Choices for Care Assessments for hospitals, skilled nursing facilities (SNFs) and in client homes. Jordan has eight years of experience working with patients and an interdisciplinary team in a SNF and held the position of Director of Social Services. The last two years, Jordan worked with Cook County Health’s managed care organization, CountyCare, completing assessments, screenings and developing person-centered care plans for members in SNFs and SMHFs throughout Northern Cook County. Jordan obtained her Master of Social Work at Aurora University and is a Licensed Social Worker in the state of Illinois.

Jordan was born and raised on the Southside of Chicago in the Beverly/Morgan Park neighborhood. Jordan’s professional background and life experiences have prepared her to help guide clients through life’s most difficult challenges. Jordan believes that every client carries a story that has shaped how they experience the world. Life is full of complexities, and sometimes navigating them can feel overwhelming. As a social worker, Jordan believes it is important to develop a collaborative process built on trust and respect with all clients. Outside of work, Jordan is a proud mom of an energetic toddler and enjoys spending time with family and friends.
